The referral pathway

One door in, the right specialist out

Every referral starts the same way: a single conversation with the unit. From there, we direct the person to the program built around their age and condition.

Referral received

Sent by a GP or specialist to the Team Leader, Neurosciences Unit.

Triage call

(08) 6159 6464 · Mon–Fri, 8.30am–5pm — discuss the referral before or after sending it.

Who we are

A specialist service, close to home

The NSU is headed by a Consultant Clinical Neuropsychologist and works with people who have, or may have, cognitive impairment or a neurological condition — across every age group, from children to older adults.

The unit is administered through NMHS Mental Health Services and is based at the Graylands Health Campus, but the service is delivered statewide on a community basis.

Some appointments can be delivered by phone or video. If this suits you better, you can ask for it when we contact you to book.

The NSU operates Monday to Friday, 8.30am to 5.00pm. We're closed weekends and public holidays.

Ages 6–17

Child & Adolescent Assessment Clinic (CANAC)

For young people with neurological, genetic, neurodevelopmental and/or mental health disorders with associated cognitive impairment.

Adult

Adult Assessment

For adults with significant cognitive impairment from medical, neurological, genetic, neurodevelopmental or mental health disorders — e.g. stroke, dementia, alcohol dependence, acquired brain injury, chronic schizophrenia.

Huntington's Disease Program

Support for patients with Huntington's disease and their families or carers, through premanifest and manifest stages. International research opportunities are also available.

Onset under 65

Young Onset Dementia Clinic (YOD)

Assessment and clinical management for a diagnosed or strongly suspected young-onset neurodegenerative condition. Referrals enter via Neurology review.

Predictive Testing — Huntington's Disease

For families with a history of HD. Run with Genetic Services WA and PathWest. The person is seen by a social worker, and may also see a sessional neurologist or psychiatrist.

Predictive Testing — Familial Dementias

For people with a family history of neurodegenerative disorders — including some heritable forms of frontotemporal dementia, motor neuron disease, familial Alzheimer's disease and spinocerebellar ataxia.

Use the referral criteria and form for the relevant program, listed under our programs above.
