Cockburn Wellbeing works with general practitioners, mental health services and community support agencies to provide ongoing care for people living with mental health concerns.
Cockburn Wellbeing is a Nurse Practitioner-led clinic established through a partnership between Curtin University School of Nursing, Fremantle Hospital Mental Health Service, and Cockburn Integrated Health Service.
We support people experiencing mental health concerns and help connect them with community health services so they can stay engaged in ongoing care.
Our services are delivered by Nurse Practitioners and nurses with experience in youth and adult mental health, chronic disease management, and alcohol and other drug support.
Karen — Director & Academic. A registered nurse with over 20 years in mental health, focused on improving physical health outcomes for people living with mental illness.
Calum — Nurse Practitioner (Drug & Alcohol). Over 35 years in mental health with a specialty in addictions and person-centred planning.
Tamarra (Tammy) — Nurse Practitioner (Mental Health). Over 20 years in mental health, with experience as an Authorised Mental Health Practitioner.
Jo — Administration & Research Assistant. Supports day-to-day enquiries and clinic operations.

Monitoring and management of chronic conditions such as diabetes, asthma, hypertension, and more.
Assessments, counselling, goal setting, pharmacological interventions, and harm reduction.
Hepatitis C management and ongoing support.

Referrals are welcomed from GPs, mental health services, hospitals, and community agencies.
